Cheung Fat Home for the Elderly
Introduction
"Cheung Fat Home for the Elderly" (CFH), situated in Cheung Fat Estate in Tsing Yi, has been providing residential care services to the seniors since 1991. In order to meet the service needs, it was converted to a 80-capacity home providing continuum of care in 2014.
With the core value of Continuing Care and People Centeredness, we actively apply the self-efficacy approach and the use of gernotechnology in taking care of seniors.
The aim is to facilitate and maintain seniors' health and encourage them to enjoy life. Also, we facilitate connection between seniors and their family members, allowing both parties to enjoy familial love.
Service Target
Applicants who aged 65 or above (persons aged between 60 and 64 may apply for service if there is a proven need) must be assessed by the Standardized Care Need Assessment Mechanism for Elderly Services of Social Welfare Department as suitable to access residential homes providing a continuum of care and mentally suitable for group life.
Application for Service
Applicants can approach respective Family Service Centres, Medical Social Service Units, District Elderly Community Centres, Neighbourhood Elderly Centres or Social Centres for the Elderly or Integrated Home Care Services Teams for referral. Arrangement of provision of service for suitable applicant through interview and assessment by The Standardized Care Need Assessment Mechanism for Elderly Services of Social Welfare Department.
Withdrawal
Residents can apply for exit from our service in writing at least one month before the withdrawal.

- Regular visiting registered medical practitioners
- Visiting doctors of psycho-geriatric service from the Hospital Authority
- Visiting community geriatric assessment service from the Hospital Authority
Personal Care Service
- Daily care service and health care
- Balanced diet and special diet for the residents
Rehabilitation Service
- Physiotherapists, occupational and speech therapists provide regular assessment and therapeutic exercise or treatment.
Psychological and Social Support Service
- Individual counselling services
- Interests, festival celebrations and social activities
Spiritual Support Service
- With collaboration with local churches and organisations, pastoral care and support are provided to residents regularly so as to facilitate their spiritual wellbeing.
Fees and Charges
Fees will be charged according to the guidelines set by the Social Welfare Department.
Respite service for the Elderly
Providing temporary or short-term residential care for the elderly in need and support service to carers. Fees will be charged according to the guidelines set by the Social Welfare Department, and applicants can be referred by district service units.
