"Journey of Fulfillment" Compassionate Community Initiative

 

Introduction

The Journey of Fulfillment Compassionate Community Initiative aims to promote the development of a compassionate community in Kwun Tong District. It focuses on three major aspects: life and death education, advance care planning, and end-of-life care for providing comprehensive support to seniors and their caregivers at different life stages and ensure every senior can walk through a regret-free life journey with the community's care.

 

 

Duration

From September 2025 to August 2026

 

 

Objectives

  • To educate the public about late-life information and reduce taboos of life and death topics in the community.
  • To mobilise community strength to promote participation among different stakeholders and jointly build a compassionate community.
  • To provide practical support in advance care planning and end-of-life care for seniors and their caregivers in need.

  • Residents aged 55 or above residing in Kwun Tong District
  • Residents with late-stage illnesses requiring End-of-life care and their caregivers
  • Community stakeholders

Service Contents

 

Connecting with stakeholders – Building a compassionate community

 

Life and Death Education

Utilising a community-based intervention model to enhance the awareness of seniors and community members regarding life-and-death issues and advance care planning.

 

"Life Companion" End-of-life care Volunteer Training Programme

 Training volunteers in the knowledge and skills related to advance care planning and End-of-life care.

 

Stakeholder Participation

Connecting different community stakeholders to jointly care for late-stage patients in the district.

 

 

Multi-level Intervention – Promoting Compassionate Community Development

 

Individual Level: Providing End-of-life care

Through case follow-up, services such as wish-fulfillment support and End-of-life care volunteer visits are provided to late-stage patients in the district.

 

Family Level: Supporting Advance Care Planning

Individual consultation is provided for facilitating the discussion of Advance Care Plan (ACP) between seniors and their caregivers. Community resources referrals are used to further enhance the advance care planning for seniors and caregivers in need.

 

Community Level: Promoting Life and Death Education

By organising "Tea Conversations", intergenerational art workshops, and informational seminars in the community, increase public awareness towards life and death issues, promoting the development of compassionate community in Kwun Tong District.
