Goal

The centre is dedicated to provide comprehensive and professional community support services for elderly people by adopting the integrated approach based on the community needs for the ultimate goal of enabling the elderly people to live a dignified and respectful live in the familiar environment; and building a community with mutual care and love.

Services Provided

With a pool of experienced and professionally trained staff and various networks, our centre will provide a wide range of direct services, as well as collaboration with and support to elderly service units in the district to cater for the health, psycho-social and personal care needs of both healthy and frail elders.

Counselling Service

To enable elderly people to deal with their problems in a constructive way and to lead a healthy and positive life.

Community Education

To promote care for the elderly people through the organization of different educational activities for both the elderly people and the public.

Support Team for the Elderly
  • To provide social networking and outreaching services to vulnerable elderly people(i.e. those living alone or lack of family support and social network, those with poor health and having financial difficulties, those living in unfavorable environment and lack of knowledge about community resources)
  • To promote volunteerism among elderly people.
Integrated Home Care Service

To assist frail and needy individuals, families and elderly people to maintain normal household function and self care ability for the ultimate purpose of enabling them to live in the environment which is familiar to them.

Target Group
  • Elderly people aged 60 or above and living within our service boundary.
  • Disabled persons including those with mental handicap, physical handicap or mental illness.
  • Individuals and families with social need.

With the varied needs of the above target groups, they are classified under two categories:

Frail Cases:

Refer to cases who suffer from moderate to severe level of impairment or disability, and require a comprehensive package of home care and community support service.

Ordinary Cases:

Refer to cases who suffer from no to mild level of impairment or disability, or those cases that are assessed at moderate or severe level of impairment but require only personal care and home-making service.

Scope of Service
Frail cases:

The Integrated Home Care Services Team would address individual service users’ health concerns and corresponding needs by applying a multi-disciplinary approach including nursing care, personal care, rehabilitative service and social work service etc. A planned and well co-ordinated package of home care and community care service tailor-made for individual service user according to their frailty and disability would be provided, including :

  • Care management
  • Basic and special nursing care
  • Personal care
  • Rehabilitation exercises
  • Centre-based day services
  • Carer support services
  • Respite service
  • Environmental risk assessment and home modifications
  • Home-making service
  • Provision of meals
  • Transportation and escort service
Ordinary cases:

IHCST would provide the following service to eligible service users:

  • Personal care / simple nursing care service
  • General household or domestic duties
  • Escort service
  • Child-minding
  • Home respite service
  • Health/safety
  • Purchase and delivery of daily necessities
  • Provision of meals and laundry service
  • Other duties --- to provide support to carers of the service users etc.
Fees and charges

It is based on a sliding scale with reference to individual or family income and may be revised annually.

Educational and Developmental Service

To enlarge the life circle of the elderly people and to add meaning to their lives through the provision of various educational and developmental activities.

Membership

People aged 60 or above and living in our service boundary are eligible to be the members of our centre-based service. Interested person can bring 2 photos together with membership fee to the centre for registration.

Health Education Service

To provide regular health checks, consultations and talks to elderly people so as to enable them to develop healthy life style.

Carer Support Service
  • To provide a variety of support services to carers aiming at enhancing the ability and confidence of the carers to take care of the elders in the community.
  • We have set up a Carer Resource Corner in our Chin Wah Day Care Centre for the Elderly.

Supportive Services

To provide basic facilities and services to support those elderly people living in adverse home environment which has no proper facilities for daily living activities and personal hygiene purposes. Such services include : Laundry Service, Bathroom Service and Canteen Service.

Canteen service is provided at the following time:

  Monday to Friday Saturday

Wan Hon Continuing Education

To provide learning opportunities and choices for those aged 50 or above living in the community. A variety of courses are provided. You are welcome to enquire the details by phone or in person during our opening hours.

All Age Drama Troupe

Objectives:

  • To develop the potential and creativity of the elderly and the youth through producing and acting out the drama.
  • To increase the mutual understanding and acceptance between the elderly and the youth through such a co-operation.
  • To enhance the self-image of the elderly and to empower the elderly through the public shows in different communities of Hong Kong.
  • To utilize drama as a means of community education and to promote positive messages to the public.
